Geez, I feel stupid. I should have waited a while before
I posted. After taking a break from the testing I realized
the one thing in common was the adapter. Sure enough, a
close inspection using a bright light to back light the PCB
revealed a fine brown 'corrosion' that looked as if it was
shorting traces. A pencil eraser fixed the problem. Next time,
inspect first, post later.
